*REFERRAL PROCESS: BT customer completes the above Refer-a-Friend form. When the new customer requests service they must mention the name of the referring BT customer.
**CREDITS & PAYMENTS: Each party will receive a $50 bill credit after the new customer maintains services for 60 days in good standing.
